Each disease category level has a risk resolution protocol which our health managers, in conjunction with health care providers, apply prior to, during and after the onset of a disease. This entails preemptive intervention, coaching, disease, and medication management, as well as self-care and home health monitoring equipment. For acute non-emergency medical problems, we provide telephonic care.
